Physicals for Your Child
At CommunityMed Urgent Care, we make it easy and affordable to get your child ready for the season. Sports physicals are free with most in-network insurance, so there’s no cost to you.* If your child doesn’t have insurance, we offer physicals for just $30 self-pay — no hidden fees, no surprises. With same-day appointments available seven days a week, you can walk in or reserve your spot online.
*Insurance eligibility may vary. Please bring your insurance card at the time of visit, or call ahead if you have questions about coverage.
What To Bring With You
- Please be sure to bring your child’s prescription eyeglasses or contacts (if applicable). Part of the physical is an eye exam.
- Also be sure to bring your copy of the sports physical form for our clinical staff to finalize and sign. You will need to make sure that you fill out the first page of the UIL form and sign and date it at the bottom.
- The insurance card which is used to cover your child (only needed for the FREE in-network physicals).
- For chronic conditions, please bring a clearance form from your specialist.
What To Expect
- Part of the physicals may include a brief discussion regarding your child’s current or past conditions, allergies, injury treatments, or medications.
- We are unable to provide or validate your child’s immunizations. Please be aware that your school may require you to include the immunization record along with the physical exam form.
- Please see your Pediatrician for high-risk physicals. Our clinic is unable to sign off on these physicals.

MONDAY – FRIDAY: 4 PM to 7 PM
SATURDAY: 10 AM to 7 PM
SUNDAY: 3 PM to 7 PM

Affordable Sports & School Physicals for Kids & Teens
- We accept Aetna, BCBS, Cigna, Scott & White, UHC, and Humana. If your plan is in-network and you haven’t used your child’s annual wellness exam, it’s free. Otherwise—or if you have a high-deductible plan—it’s just $30.
- Our physicals are for youth ages 7-18.
- Post-COVID-19 evaluations or ECGs desired for additional cardiac screening need to be provided by your pediatrician or cardiologist.
- We are unable to complete Special Olympics physicals.
