For chronic conditions, please bring a clearance form from your specialist
After You Book a Spot
- Complete registration paperwork online (part of booking your spot)
- Bring COMPLETED UIL/Athletic or other physical forms
- After your first initial physical form, additional forms are $10
Wellness Exam Includes:
- Physical Examination
- Health History
- Height/weight/BMI/blood pressure measurements
- Vision Check
- Age appropriate developmental guidance
- Depression Screening
*The Free School Physicals with In Network Insurance will be your child’s more comprehensive annual wellness exam, which is free with in-network insurances. We will INCLUDE the school physical and paperwork with this exam. If you have already used your child’s annual wellness visit this year, then we will bill you the $49 for the physical.
